Speaking as a longtime rash sufferer (various places, albeit quite sporadic), I've eventually concluded that it's closely related to my sleeping habits: it gets significantly worse when I'm having particularly severe insomnia. Which may mean that it's stress-related, of course -- the correlations are tricky.
I mostly control it with Claritin, which is pretty effective and has fewer side-effects than Benedryl in the long run. (Benedryl can be habit-forming if you use it too much -- I learned that one the hard way.) And it's not too horribly expensive if you use the generic knock-offs -- I actually use Alavert, which is about half the price of brand-name Claritin.
I've also got a topical ointment which I use when it gets *really* bad, but this generally hasn't been needed since I started on the Claritin...
